Secure Web Portal for Referring Providers

SGMC Health offers a secure, web-based portal, EpicCare Link, that allows referring providers and members of their care teams to submit referrals and view relevant patient information in real time.

This platform is the primary way providers can refer patients to SGMC Health services and communicate with SGMC Health providers, helping ensure timely coordination and continuity of care.

Use the referral portal to:

  • Submit patient referrals electronically
  • Access relevant clinical information
  • Communicate securely with SGMC Health providers and services

Priority Referral Options

If you prefer to refer by phone or fax, SGMC Health offers additional options to support your workflow:

  • Priority Referral Line:
    Call 229-433-3022, Monday–Friday, 8am-5pm
  • Referral Form:
    Complete the online referral form and fax it to 229-433-3023

Transfer Your Patient

24/7 Patient Transfers

To transfer a patient to any SGMC Health hospital, providers can contact our transfer center 24 hours a day, 7 days a week.

One call connects you with a team that can help coordinate the transfer and ensure a smooth transition of care.

SGMC Health owns and operates its own ambulance service, allowing for seamless coordination of patient movement when needed. Non‑emergency transport can be requested through the transfer center as part of the transfer process.
